Having received Jesus Christ as my personal Lord and Savior and having been baptized by immersion, and being in agreement with the statements, strategy, and structure of Christ Image Church,I now feel led to unite with the Christ Image Church family.

Because I realize that I am a new creation in Jesus Christ, I want to live for him out of my identity as a child of God.

Therefore, I commit myself before God, before the body of Christ at Christ Image Church,and before all those who are witnesses or reviewers of this covenant decree,with God’s help to do the following:

I will CONNECT

with the Christ Image Church family.

I will SERVE

in the ministry of my church.
